This one's image quality is vastly different from the rest of Carlton captures at the time, because this is a modern digital scan of a breakdown slate. Whenever there's some problems on the programme (most likely sound issues), the slate would be used to overlay an apology message to the unsatisfied viewers. 

Given the age of their initial transmission, however, there hadn't been a recording showing such an occasion happening on screen, only this slate found in clearing out their studios decades later.
